Warning on Exjade and Liver Failure (March 2008)

Novartis is warning healthcare professionals that hepatic failure can occur in patients treated with Exjade(deferasirox). Exjade is used to treat tranfusional hemosiderosis, the chronic iron overload that can result from multiple blood transfusions.
Cases of liver failure, some of them fatal, have been reported in patients treated with Exjade. Most of these cases occurred in patients over 55 years old who had significant comorbidities, including liver cirrhosis and multi-organ failure.
Because of this, liver function should be monitored monthly, and if there is unexplained, persistent, or progressive increase in serum transaminase levels, Exjade treatment should be modified or interrupted.

The Iron Council (Assembly of Iron) is the 6th boss in Ulduar, and is actually three bosses. The order in which they are killed determines the difficulty of the encounter. For this, we did easy mode (Steelbreaker, then Runemaster Molgeim, then Stormcaller Brundir). Fusion Punch is a nasty debuff Steelbreaker puts on someone (ideally the tank); cleanse this ASAP. While Steelbreaker is being tanked, the other tank needs to be on the other two bosses, avoiding Overload as it will easily kill him. Once Steelbreaker dies, separate Runemaster Molgeim and Stormcaller Brundir. Have a rogue with Brundir to Kick him as much as possible, as his abilities REALLY HURT. Runemaster will spawn Death Runes (big green runes on the floor) around a player; get out of the way! Once he’s down Brundir will start to do Lightning Tendrils, lifting up off the floor and tracking someone. Have that person get away from the raid. Also, once he lands his threat is wiped, so tanks will need to taunt as soon as he comes back. uming you can survive all this Iron Council is a dead set of bosses.

where it was demostrated that some iron-enriched foods actually contain enough elemental iron to be magnetic. I wanted to see more of this, so I did the experiment myself, and sure enough – even the cereal I had for breakfast was magnetic!
In this video, you’ll see floating magnetic Bran Flakes, a free-standing magnetic Bran Flake demonstration, and finally, the actual magnetic iron material extracted from the cereal and shown under high magnification.
This sort of iron looks similar to the kind of black iron you would collect by passing a strong magnet through ordinary soil. After a bit of reseach, it turns out that this “reduced iron” will dissolve in your stomach’s hydrochloric acid, at least to some degree. It can be then absorbed into your bloodstream for bodily processes, although not as efficiently as iron compounds such as ferrous fumarate, iron sulfate, “chelated” iron/amino acid compounds, or colloidal iron found naturally in plants/non-processed foods.
A quote from an iron ore mine (manufactuerer) press release in Peru:
“Direct Reduced Iron (DRI) is a high-quality iron product created through the removal of oxygen (reduction) from iron oxide material, in solid state (without melting). Reduction is achieved when a reducing gas (typically hydrogen and carbon monoxide) is passed through the iron oxide feed material at a temperature between 900 degrees and 970 degrees C. Direct Reduction Iron is commonly used in an Electric Arc Furnace (EAF) to produce steel, instead of, or in combination with, scrap iron.” (Thanks to YT user BadAtmosphere for the info)

New Warnings on Exjade (Aug. 2007)

Novartis has notified healthcare professionals that new warnings have been added to the labeling for Exjade (defarasirox). Exjade is used to treat transfusional hemosiderosis, the chronic iron overload that can result from blood transfusions.
Cases of acute renal failure, some of them fatal, have been reported in patients taking this drug. Most occurred in patients with multiple co-morbidities who were in advanced stages of their hematological disorders.
Before beginning therapy with Exjade, all patients should have their serum creatinine essed in duplicate to establish a baseline level, and then creatinine should be monitored monthly during treatment. Patients who may be at increased risk of complications should be monitored weekly during the first month of Exjade therapy or when the treatment is modified, and then monthly thereafter.
These high-risk patients include the elderly, people with pre-existing renal disease or other co-morbid conditions, and those receiving drugs that can depress renal function.
There have also been reports of cytopenias in patients on Exjade, some of them fatal. The relationship between the drug and these effects is uncertain, since most of the patients had pre-existing hematological disorders that themselves are ociated with bone marrow failure. Blood counts should be monitored regularly and treatment interrupted in patients who develop unexplained cytopenia.
